The following forms are needed to process a Contractor's License Report.

No need to mail a check, as a "Check Authorization Form" is included to
process your Contractor's License Report. As soon as you Submit the
following forms, we will start processing your credit reports and will have
them processed the same day if received before 12:00 noon.
If received after 12:00 noon they will be ready by noon the next day.

The charge is $50 for each report needed. If both a Personal and Business
report are needed, the charge will be $100.

Also fax a copy of your Check marked "Void to: (954)771-1777"
