The following forms are needed to process a CLAIM OF LIEN.

No need to mail a check, as a "Check Authorization Form" is included to
process your Claim of Lien. As soon as you Submit the following forms, we will start processing your Claim of Lien.

Be sure to also fax a copy of your Notice to Owner (if one was done), and
also fax a copy of your Check marked "Void to (954) 771-1777".
